Registered Nurse

  • Offer Qualified Professional observation and assessment, and report to the County as appropriate
  • Train, supervise, and consult with Personal Care Attendants, companions and responsible parties/guardians for quality of care
  • In-home Client visits, which include client education on in-home personal care services
  • Develop personalized plans of care
  • Provide case management and advocate for clients to ensure the highest quality of care at home
  • Act as a liaison between (families, guardians, doctors, and community workers)
  • Arrange for care and coordinate with other providers (e.g. Medical Equipment Suppliers, Pharmacies, Medicare or Hospice)
  • Are on-call 24 hours a day
